The only way I can imagine causing such a thing is having a relatively high pressure in your tapwater or very deep well water and performing a large water change wirectly from the tap/well. I regularly do weekly 50% changes straight from my tap and have never witnessed such a thing…

I wish I could help, I don’t believe I’ve seen such a thing myself.
